FRIDAY: 8:30 am - 3:00 pm (By Appointment)

Closed for lunch from 11:45 am to 1:00 pm. Closed Saturday, Sunday and federal holidays.

For National Guard Soldiers:

Valid photo ID.

DD Form 1172 verified by unit's verifying officer.

Enlistment contract or orders stating ETS date.

Promotion orders (if applicable).

For National Guard Dependents:

Spouses: Valid photo ID, marriage certificate and social security number.

Children (up to age 21): Valid photo ID, birth certificate and social security number.

Children (age 21 to 23): Birth certificate, social security number and letter from attending college stating dependent is a full-time student and expected date of graduation.

For Active Duty Dependents:

Same as above, with active duty orders. (Copy of orders can be obtained from 1/69 S-3 shop if you don't have a copy.)
